Rankone & Echo-cardiograms

All Athletes must fill out required Rankone forms.  Please see the campus athletic coordinators for access to these forms.

ECG Clarification: If you put yes to the ECG, then you have to either go to a doctor and have one done then submit to trainers a doctor's clearance note OR refill out the medical history form in Rankone and answer NO to the ECG.

Returning Athletes 

Each student should have a physical on file for the current school year. It should already be completed, cleared by the physician and checked/entered by the middle school coach or high school athletic trainer.
